Hi P,
It is perfectly OK to type the additional information. Indeed, if you prefer you can type all the answers and add them at the end of the form. If you do this you should make sure that your name and NINo is at the top of each sheet and that the bottom right corner is marked with the page number. We suggest that you mark '7 of 12' etc as this makes it clear if anything has been lost from the end!

One other thing. On supporting letters, ie from my GP, who do they address it to. Would it be just a dear sir/madam.

I have another question
When answering the question about how for can you walk does this mean how far can I walk repeatedly. So if I can walk 20mt but then can't do it repeatedly on a regular basis what answer should I give. Hope this makes sense.

If you feel that none of the tick-boxes accurately reflect your limitations, then leave them blank and explain in the text box.
If you could not repeat an action within a 'reasonable' time-scale, it is better to explain this from the start.
