Hi I am in the support group, following severe affects following radiotherapy 12 years ago, that cured my cancer but has left me unable to work due to the side effects of the treatment, resulting in ongoing chronic pain and more recently necrosis of my jaw. I was also subsequently diagonosed with colon cancer in 2013, which while treated, has created other side effects. I see that there is a question on the form to declare my condition relating to post cancer treatment and also requiring me to get my GP to complete part of the form.
Is there any advice as to whether I should any more than tick the boxes on page 4, declaring post cancer treatment, leaving the detail to the GP?
Has anyone else received feedback from completing this questionnaire, following complications from cancer treatment?
Any advice would be very welcome.
