You need to contact the Assessment Provider to see why they have not gone ahead with a Paper Based Review.

I am very anxious about having been given an appointment for a home assessment and my CFS/ME symptoms have been very bad since I received the letter yesterday, so may I ask you a further questions which may help alleviate my symptoms over the bank holiday? If they do insist on me having a home assessment what happens if I am not able to answer all of the questions due to my fatigue/brain fog/anxiety/possible panic attacks? Will not being able to answer all the questions affect my chances of getting a good award? How much weight is placed upon the face to face assessment as opposed to the filled in questionnaire? I am confident that following the guidance given on this website my questionnaire is filled in very well. I may be able to push myself to answer all the questions but this may result in a deterioration in my health needing several weeks recovery time, so I am unsure whether to do this or not. Can you also tell me how long the assessment takes and if I will be allowed to take a break? And will the HP be able to view my questionnaire during the home assessment? It says in your guide that they cannot access PIPAT during a home assessment.

If you can't answer the questions then you can't, I would encourage you not to worry about things that you cannot control, I know it's easier said than done, but it's worth the effort.
If you can't answer a question then just explain why you cannot, if memory is one of the things that limits your ability to complete the PIP activities, then it's confirmation that you have problems in this area.
I'm unsure whether the assessor has access to your report on their laptop or not, this section of the guide was written when PIP was first implemented and the facilities available to an assessor may have been updated. However, they are unlikely to be referring to it during the assessment as they follow a script, it is after the assessment when they are writing up their report that they will look at your form.
Assessments are usually about an hour, the assessor won't be keen on taking a break as they may have other appointments.

There's no information available about how answering the assessors questions or not translates into an award, just do your best.
It's still possible that a PBR will be done, you'll have to see what they say when your partner rings next week, but if they say no then I think you will have to resign yourself to attending.
