Is there anywhere online that I can get an up to date sample of the whole pip2 form? I don't mean to apply online I just mean so I can see exactly how the form is set out. I have not sent off for mine yet and it would help for me to have this so I can plan a bit before they send it to me.I know you have the guides but I need to see the form itself.
Also in the questions where they give you points are there actual tick boxes at the side of each descriptor or just do they decide it themselves from how you describe how it effects you?

There is a sample PIP2 on the following link, however, it dates from 2013 and there have been cosmetic changes to the current form although the content is the same.

Some of the questions on the form are tick boxes, but there is nothing specific on the form that allows you to indicate which Descriptor you think you meet. The software used by the assessor to produce the assessment report allows them to select the Descriptor they are going to recommend to the DWP.
