This activity is only about problems with the bladder and the bowel so I don't think you have missed an opportunity to score points, especially as you say you have no problems with the toilet.

Your form and anything attached will scanned and then forwarded to the Assessment Provider covering your postcode, they will decide whether a face to face assessment is required or not.
