. Throughout the decision makers letter explaining his reasons for finding me fit for work it keeps referring to the esa50 questionnaire I had completed then goes on and states esa50 questionnaire sent but not returned. in mandatory reconsideration letter states that original decision maker did not have access to my esa50 questionnaire and can come to a decision based on the HCP report only. The decision makers letter was full of mistakes and inaccuracies and even failed to put a name on letter. My question is can a decision maker come to a decision on being found fit for work based on the HCP report only.
