You have two options.
You can be assessed to see whether you are eligible for Income Related ESA, if you are then your payments should continue without interruption.
Or, you can in principle make a new claim for Contribution Based ESA. However, you will need to re-meet the two Contribution Conditions from scratch and one of the years used must be newer than the years used when you originally claimed.
The two Tax Years that will be used are 2015-16 and 2016 -17. You will need to have 50 weeks of NI Contributions or NI Credits in both years and 26 weeks of NI Contributions (from working) in one of the Tax Years and in the case of the latter it must be newer than the years used for your current claim.
