ESA50 Form - "Moving Around" Distance

ESA50 Form - "Moving Around" Distance was created by cmt
Hello there. I am completing my ESA50 form (for a renewal). I am currently in the Support Group of ESA and am stuck as to how to answer the ESA50 form in order to qualify again for the Support Group.
Sorry, that's probably not clear...
EXAMPLE: The tick boxes for "Moving Around" distance have options for:
- 50 metres
- 100 metres
- 200 metres or more
- It varies
I use a powerchair (electric wheelchair). Without the powerchair I am unable to move more than around 15 metres (regardless of what aids used).... I can provide evidence of this but I don't know which option to tick as I clearly cannot mobilise anywhere near the "shortest" distance listed on the form?
Maybe I'm interpreting the choices wrong? Help is appreciated.

You do not need to use the tick boxes although you must answer the question, so I would cross them out and mark them "see below" and then explain in the comments how far you can mobilise, don't forget that ESA also considers you ability to self propel a manual wheelchair, so you need to include details for this as well.
