pip claim form was created by amanda lowe
I filled in my pip form lat November2016 from dla to pip I was on middle rate care and low rate disabilities. Then when cab filled my pip form in I only got standard rate daily living and standered mobility. Ive got another form after I told them of a change coz I got diagnosed with another too disabilities. Please can u help me to fill the form in correctly. Thanku Amanda lowe
