ESA POID1 form was created by Don
I received the paperwork for my ESA tribune and noticed that they have lost my POID1 form. Is this a problem or not. I understand that this form is proof of identity but don't understand what information it contains.

It's completed by the reception staff if you attend at the Assessment Providers offices or the assessor for a home assessment and basically lists the documents that were provided to validate that you were actually the claimant.
