I'm afraid I'm not sure what value it would be to your ESA, the criteria is very different and it's age means that it is unlikely to be reflective of your problems as they are now.
Have you looked at our ESA Claim guides?

Thanks Gordon for your reply,i understand it is different now with the ESA form,my reason for wanting to enclose it is it covers a one part of my condition that has now deteriorated making me basically housebound. That apart I have many other conditions now that I thought it would be supportive,my GP never told me till I asked why my back pain is severe. He told me I have Spondylitis.Spine Abifada from birth.I have been filling the form out using the site and it's been very helpful,my last face to face was in 1999 but have got medical evidence for all my problems as my GP sent me to hospital with an envelope which I opened and it was 5 pages of my medical history lol, first stop was photocopies which I will be sending to back up what will be in the form. Many thanks and will update when hopefully I stay in the support group..

You would be better of using the medical evidence that generated the Tribunal result rather than using the result itself.

Thanks for your reply,i have found some GP letters from the Tribunal which were a major part of the decision and are consistent with my deterioration in health. Will think what you've said and will let you know how I get on.
