Hi there I already receive PIP enhanced rate for both daily living and mobility and did continue to work with adjustments and reduced hours over the past 4 years. It has got to the stage where I am no longer able to carry on and since August last year I have been off sick. My SSP will finish on 13th March and I am trying to complete the initial ESA1 form to start my claim. Please could you help with queries
Do I send in any medical evidence at this point?
On pg 10 it asks "are you going back to work" - should I tick NO or would this have repercussions?
There is a blank page for "Other Information" but not about your disability, illness or health condition so what information are they looking for?
Also, as I am applying for ESA will they look at my PIP award again even though it was awarded from 2016 to 2026?

We can't really help with your employment status, speak to your union representative they will have all of the information that you need. Do not resign without getting advice first.

You don't need to send any medical evidence with the ESA1 other than a Fit Note covering you from the date of your claim, make sure you keep a copy.

You will need an SSP1 certificate from your HR department which states that you have exhausted your Statutory Sick Payments.

Once your ESA claim is accepted you will be sent an ESA50 form that asks you about your health problems, see our ESA Claim guides for more information.

Whilst the reports from your PIP claim can be used for your ESA claim we have only seen this done in a positive way, to support an award.

Hi Gordon many thanks for your reply.

Regarding my question on what to put on the ESA1 form, it asks you whether you are returning to work or not. I don't intend to resign as I am on long term sick and still classed as an employee. CAB advised me not to resign but instead wait for the inevitable of being made redundant. I just wondered if I go ahead and put NO against the question of returning to work on the esa1 form, whether the DWP would contact my work and advise them I am not returning and assume I am resigning?? Sorry if it sounds like a silly question, my brain isn't the best.

Also, re the blank page near end of the form 'Further Information', would you know what it is they are looking for if it is not health info they want?

Sorry that I missed your other questions.

No they won't contact your employer, I'm actually not sure why they ask this as I am not aware of the information being used at any other part of your claim.

The "Other Information" page is for anything else that you think that is relevant to your claim, if there isn't then you don't need to use it.
