Sorry to be a pain Gordon i bet you hate me already ha.

Anyway i emailed the people that conduct the assessments i just asked them how i should send the GP note to them, They said by email and post is fine. Just wanted to run it by you before actually doing it, Do you think sending by Email is ok?

It would help if you could your posts in one topic so I didn't have to search for first half of this conversation! :)

Do both as suggested, the email will get there the quickest so they will have time to decide on whether to grant a home assessment and for you to deal with it if they don't.

Ok noted Gordon i will post in one topic now thanks.

Thank you for you reply and i will take your advice and do both, Have a telephone appointment with my GP Friday to ask if she can amend the letter she did for me. If she refuses then i will just have to send the original.

Spoke to my GP today and she has agreed to do me another letter and will remove the part where she says that i can go somewhere that is familiar also she has agreed to put more detail in the letter. So hopefully that will help

Hi Gordon hope your well, Today i got my GPs amended letter i just wanted to put it on here to run it by you. I have already emailed it accross tot he assessment place and will send a copy through the post also. Letter is below -


Mr T***** is a patient at this practice and is under my care. He suffers significant symptoms of Anxiety, Depression and Agoraphobia. He finds it very difficult to travel out of the house. He also has difficulty in day to day activities and self motivation. He recently deteriorated significantly with concerns regarding suicidal thoughts.

Mr T***** has been assessed by the mental health, Acute crisis team. He remains under their supervision and has been allocated a Care Co-0rdinator. He has been attending a clinical Psychologist for the last 6 months. His medication is below -

Mirtazapine 15mg
Olanzapine 5mg
Pregabalin 100mg x2 Daily.
Venlafaxine 75mg x 3 Daily

I would request that any Welfare assessments are conducted in his home environment to prevent an exacerbation of his symptoms. At this present stage he is being closely monitored by primary care and mental health services, As he remains at high risk of deterioration.
