I am seeing my doctor today is there a form I need to get her complete or should I take something with questions for her to answer ? I have been advised to apply for pip for first time should I wait for esa decision first feel very overwhelming.

For PIP we have a GP Notes document that they can complete or use it as a template for their own letter.
