I reported a change in circumstances mid-December, expecting to be sent an AR1 form to fill in? Instead I have just received a very lengthy form called a PIP.1021 which at the bottom of the Information booklet is dated Feb 2015. Can someone tell me if this seems to be the right form for me. I have a number of health issues, but I reported only one change. This was an improvement in mobility? Thank you.

The PIP.1021 is just the covering letter, you should have received a new PIP2 to complete. This is standard for when the claimant reports a Change of Circumstances.
