ESA50 Form

I have had my form filled in by someone else and they have made a mess of it. Can hardly read the writing. Can I use the downloaded PDF form and fill it in again by hand...

Yes, or you can type your answers and attach them to the new form

should I send the forms back loose and in order or should I staple them together.

Attach the pages to the back of the form using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.

Do you mean staple the pages to the back of the original ESA50 form. Sorry my head is all over the place at the moment and not sure what you mean.
