Hi, How do I obtain a copy of my medical report from the Capita assessor?
I was previously given lower rate DLA for both components for an 'indefinite' period. Now after my application from DLA to PIP I have been given 8 points for Daily Living and 4 points for mobility so will only receive the lower rate of Daily Living and nothing for mobility.
I suffer from MS, COPD and Anxiety/depression...which can make it extremely difficult to go out and meet people and converse. It's also difficult to walk for any distance or time due to the pain from MS and the breathing difficulties from COPD.
How do I go about getting a copy of my assessment?
