I have just received a ESA50 questionnaire to fill out. l am a little confused as it states on the front do ’NOT Ask or pay for any new information from your doctor and only send in what evidence you already have. I assumed the more information you sent the better? I'm unsure what to do? I'm a little worried they will use this against me. Any advice would be greatly appreciated.
