ESA and GP letters

Hi there, well after 6 years Maximus have finally got around to wanting to do a review which I'm dreading. I've been in the support group for over 12 years now, and I have some pretty unique problems which I won't bother you with right here.
Thing is My GP has written me a short support letter, confirming what ails me, but in very broad terms as he's only been my GP for just over a year now. My previous GP knew me a lot better and that used to come across in what she used to write.
So as well as his recent information can I include past letters from clinics and my old doctor who goes into far more detail about my issues. Some are over 6 years old though which I know may be a problem.
Thing is for my condition is incurable and treatment options were always very narrow as I cannot take any meds bar one for blood pressure (thank god). They can't do anymore to help me, (which is stated in one of my GP's old letter written in 2011) so imo what was written still stands.
So are they worth including with a covering note?

You can use the old reports and letters but you will have to explain in detail why they are still relevant to your conditions and limitations as they are now.

So for me It would be saying that all the treatments I've undertaken have done nothing for me, and that in fact my condition has either stayed the same or in some cases worsened?
A GP letter written in 2011 says that and imo It still applies which is why I'm keen to add it to my notes.

It will help to provide some background to your conditions so that those assessing you can see that no improvement or even a deterioration is consistent with the prognosis for your conditions.

I'm not sure how I phrase that?
I'll be covering in my notes how long I've had these illnesses, how they effect me and the fact since the get go I have not been able to take any medications without causing my already inflamed bladder to be on fire, causing my bladder pain to get a thousand times worse. Sadly It's been like that since day one and there is no cure. We've tried the limited treatments available to me, but without success.
And to top It all I went on to develop Uterine cancer and heart failure due to undiagnosed high blood pressure that still isn't properly controlled even now, due to not being able to take the correct meds.
That type of thing?

Even if you are assessed by doctor, you cannot assume that they will have knowledge of your conditions, the DWP's view is that all conditions, with a limited number of exceptions, are capable of improvement and claimants can adapt to their limitations.
As much of your evidence is old it's important to show that change is not expected so see if you can provide some links to information on your conditions and in particular the prognosis.
