ESA 3 form

hi everyone it has been a while since being on here now sent a esa3 form to file in dates between 2012-2016 how am i supposed to remember i am one of those that lost money when contributions esa ran out and lost my money from IB transfer i have mental health problems and i give up fighting them in 2016 dont have a clue how to file this form in any ideas thanks

you could see if you can get an appointment with your local CAB, or welfare advice service

or even perhaps your mental health team, if you are still under their care

don't be put off by the length of the form because for many/most of us, lots of it does not even apply
