DLA to PIP no form

DLA to PIP no form was created by chinwag
I filled in the PIP1 over the phone nearly 3 weeks ago and have recieved a request for ID but not the 'How your disability affects you' form.
I'm currently in the Support group for ESA but only after a Mandatory Reconsideration that was successful in September after 7 months of no money except my DLA..
I am concerned that if they don't send me this form they will rely on the initial assessment that went badly and not the the reconsideration,
Has anyone else been in the same situation? Is it usual to ask for ID before the second form is sent out?

It's unusual for ID to be requested, especially if you have an award of ESA but you need to provide it as the PIP2 will not be issued until you do.
It's actually quite unlikely that they will use any information from your ESA but even if they do this will only occur after you have returned the PIP2 and most likely had a face to face assessment.

Thank you so much. You've put my mind at ease and I'll send the ID Monday.
My Mum answered the questions on the phone after I'd said she could speak for me so maybe that's why they want ID.
