There is no legal requirement for a form to be issued as part of either an ESA or PIP assessment.
Who is the letter from? If Maximus or CHDA or HAAS then it is for ESA, if IAS or Capita then it is PIP.
You do have the option of completing an ESA50 if time allows or of writing a letter explaining your current limitations in a format similar to the ESA50 or the PIP2.
You also have the option of cancelling the current appointment providing it is considered to the first one sent but you will have to attend the new appointment whenever and wherever is arranged for,
