My Step Son is on Indefinite DLA but will of course need to transfer to PIP at some point, is the form that's used able to be filled in online & printed off like the ESA50 forms?

Also he doesn't have a main health professional as such who see's him on a regular basis, but we do have assessments from Consultant Neurologist, Social Worker, & letters from PA Carer, & Social Worker, Day Centre Manager. So will want to include details of all these at the start of the claim so they have the full weight of opinion before them, hopefully in order to avoid the need of a F2F, should I therefore just put his GP as the Main Health Care Professional & give all the other info later in the form?

Hi Taggy

The PIP form is not available online like the ESA forms. When a letter is sent starting the transfer from DLA, a claimant has to ring the DWP and they fill in part of the form before sending it through the post.

I would list all the people at the beginning of the form and then reference their letters/reports throughout the form. (They are unlikely to contact any of them anyway - even the GP).

Thanks BIS, if the boxes for information are anything like as small as the one's on the ESA50 i'll need far more room to include information, which is much more legible typed, so would the best approach be to just type everything up on to separate sheets in Question Order, Staple these to the PIP form & state in each box that all the information is on the additional sheets?

It seems that DWP really want you to be able to put as little information as possible in the forms they send out so as either to justify refusals or forcing a F2F as often as possible. Many Thanks.

Hi Taggy

Everyone does it a little differently, but yes you can just write in the boxes "see additional sheet No X Appendix Y), and type up the rest of your evidence.

I often put a quote in the box from my evidence. So something like: "Unable to prepare a basic meal because of severe arthritis and poor manual dexterity skills as noted in DR Y's letter dated 3rd May 2019. See additional pages 3 - 8 for further evidence."

Obviously, on the additional sheets, I go into far more detail, but I like to remind them the whole time of my expert evidence rather than them reading it all at the end and having to work how it relates to the PIP criteria.

Thanks BIS, yes that's roughly the way I've done it for the ESA50 form & have frequently mentioned where the expert evidence supports this. Seems crazy the way DWP operate, some benefits/forms requiring manual filling in, others online & others initiated by telephone, a totally inept & dysfunctional department.
