GP's do not know me!

Posted my ESA questionnaire yesterday, now facing DLA to PIP transfer. Both ESA & PIP asking for GP's name etc. The 3 GP's who actually, really know me, in person have all retired, my practice of 30 years has merged with another and has turned into a monster Super Hub. Unable to book GP appointments, cannot get through on phone and cannot just 'pop in' as too far to drive.

How should I address the GP question? I do not know any of the numerous GP's and they certainly do not know me and I am sure will not have the time to look at my challenging, complicated medical history. Any suggestions please gratefully received. Thanks

Hope this helps. I’m currently in DLA to PIP transition ☹️ I’m in the same situation, what CAB advised was to add on a separate sheet of paper my situation regards lots of GPs in these big hubs, & that I have not been allocated a named Doctor. Plus I phone Ed the surgery and got them to add a note on my records about DWP and PIP enquires regards myself. At least the surgery is aware, how much good that is I’ve no idea. I also got all my hospital records, sifted through relevant details and highlighted these, and added to my claim. Remember to add your name and reference on medical evidence and letters.

At our GP surgery we no longer see a particular Dr but on our prescription is has "named Dr" of whom I have only seen once! But they have access to the medical records and will pull information off them.

Thanks Sonia. I agree that the GP's can look at my medical history, my concern is there's 29 years to plough through and I know that they don't have the time. I've purchased all my hospital notes from the 3 hospitals I have received ongoing treatment from and included a chronology for my ESA questionnaire. Hello Gordon. Ta for reply, I should have clarified. I can go online and book an appointment if I need it in 8+ weeks time. Or if I want a pill check, a smear, blood pressure check, baby check, well women check etc etc etc I would be spoilt for choice as these are provided by Health Care Assistance which there appears to be an abundance of. They do have a first come first serve arrangement but I am unable to join the queue which apparently snakes around the whole building and down the street and through a carpark. As for the phone, if you don't get through by about 08.15 all the available appointments are gone. Sorry I'm not ungrateful for the suggestions but my nice, smaller practice was swallowed up by this leviathan of a SuperHub and it has no heart or soul. Just anxious I suppose because I'm now facing a transition from DLA to PIP, what's not to like. Thanks again Gordon.
