I hope that this may help you for DLA to PIP and ESA!!! I keep updating my daily life and how my disability affects me on a frequent basis so that I can get dates, tests and other issues that are important, I then get the GP practice to scan to my records so that any GP can see how my disabilities affect me. I have a lot of medical problems and very rarely go to my GP as it is so hard to get appointments but in the last year I had 51 hospital appointments for various problems and I always request a copy of the letter sent out to my GP. This is useful evidence that can be sent in support of your claim.
I am currently waiting for my DLA to PIP decision so I am hoping the hard work in trying to describe my disability and trying to keep it up to date with all the supporting evidence will help. I struggle to remember dates so this helps with this.

Evening Julie. I appreciate your answer. I purchased all my hospital notes from the 3 hospitals I've been treated at and have used records from them to support my ESA questionnaire that went in this week. I've arranged them in a chronology from start to finish that hopefully, when read in order of the questions. I've just found the whole process daunting and I suppose I'm just out of practice, my transition from Incapacity to ESA was in 2013 and i know have transition from DLA to PIP, my DLA was back in 1999 so really out of practice. I'm just completing the 28 page PIP-1 form to get the ball rolling. So, fingers crossed, if I've done the hard work for my ESA, most of that information will transfer, give or take, to my PIP-2 application. Fingers crossed. Sue
