New Style ESA Application

Just after some advice I have recently had my DLA changed to PIP and received enhanced in care and low for mobility. I receive the PIP for a few problems including spinal problems, POTS and arthritis.

I currently work as a part time medical secretary but have been off sick since March following failed shoulder surgery. My SSP runs out on Monday so now have to apply for ESA. My current sick notes states shoulder pain following surgery.

My question is when I come to complete the ESA medical form I will also have to put in all my other health problems but will they only take into account what is on the sick note or will they look at all my other problems when deciding which group I should be in. As well as the shoulder I have also been diagnosed with another problem with my hand but this has not been put on the sick note.

Thanks for your reply yes I understand that I must complete all health conditions but my sick note says just about my shoulder so will they also take into account my other problems?
