Did you send the form using a Tracker post or a get a Certificate of Posting?
When was the return date for the form?
The DWP should have written to you in advance of their closing your claim to ask why you had not returned the form, did they do this?
Was this a new claim or a reassessment?
Start a new claim for PIP as soon as you can, if you overturn the current Decision then the two claims will be merged.
