If you are using a mobile phone then there should be an app that you can use to record the call, if you take it hands-free then you can use any recording device by placing it next to the phone, it is a landline with no hands-free option then you will need a specialist microphone to connect to the earpiece,
b. The DWP has no in-house medical resources they do not complete any part of the assessment report.
c. It could be either.
d. I'm afraid I can't remember which way around it is but one the Assessment Providers has told their assessors to read the form and evidence before the interview and the other afterwards.
