I'm sorry to say this is not a mistake. The DWP have sent out extension letters to help with the backlog, but reviews are being processed and coming out at any time. Yes that is the form, but you can add additional pages if you want. Make sure that you check out our guide for filling out PIP reviews if you're not familiar with doing one.
