Unfortunately, the DWP is inconsistent so it's difficult to know how sympathetic they will be. By the way, you don't have to put an exact date on when you were referred - just give the year if you don't know.

Hi Apple Charlie, I think that is a good idea, they can then ensure they look at all your evidence. If you have anything that DWP have already that you wish to reference, I would personally send it again ... hope this helps and good luck

My experience was that NHS physiotherapists will never put any thing in writing. But GPs can be helpful.
