Because you contacted DWP about a change of circs, DWP have sent you a review form.
You do need to get the form back to DWP by the date on the form, if you need extra time contact DWP and ask.
Did you keep a copy of your previous form? if you did you can use this as a template, don't forget to add your new medication. Have a look at our PIP Claim guide for a better understanding of the criteria you will need to meet.
