It sounds as if you have received a PIP2 rather than the AR1 Review form, this is usual where the Decision was made by a Tribunal.
WHilst it will add to your task I would include the information sent before and especially any that was sent as part of your appeal as I would not expect the latter to be available to those assessing you.
