Hi

Can anyone tell me how to request a copy of the assessment ?

It started off as a phone call and as I struggled with it I was left to rest and referred back to paper based, so how do I request a copy of this assessment and what do I ask for please?

Welcome to the forum.

You might want to have a look at the following FAQ which explains where everything is; www.benefitsandwork.co.uk/guides-for-claimants/faq/forum

You not told us which assessment you are talking about, so I will take a guess.

If you have had a PIP assessment over the phone the you need to contact PIP helpline after 48 hours and ask for a copy of the assessors report called PA4, if you had a paper based assessment then you need to ask for a PA3.

If you had a WCA assessment then you need to wait until you get a decision and then contact UC helpline and ask for a copy of the assessors report called a UC85.
