You may need to have a one-off meeting for identification or to sign your UC Claimant Commitment.
Your ESA award will be transferred without reassessment but this manual process can get out of step with the UC process, it would not hurt to contact your Work Coach if an appointment is booked to make sure they are aware of your ESA award.
If you do have a meeting then there should be no need beyond courtesy for you to explain your conditions and limitations, the Work Coach will not be medically trained and does not participate in the assessment process in any way.
