Your deadline is quite tight, I would contact PIP and ask for an extension so that your claim is not late.
We cannot give you a delivery address as it depends where you live.
When you phone for an extension ask the operator for an address with a post code, without a post code you normally cannot send any recorded delivery post. we would advise to send any mail to DWP by recorded delivery as they do loose some mail.
