Good afternoon we want to double check to draft out a form if it relates to us. If you are on ESA Contribution based does that mean the form you fill out now is the WCA50 FORM?

The ESA work capability assessment form was called ESA50 form
The UC work capability assessment form is called UC50 form
Both forms are the same apart from the name; ESA50 - UC50

So has the ESA50 Form been replaced to a WCA50 Form? Reason for asking is we like to prepare and boxes are a lot smaller on WCA50 form so if ESA50 form has been changed to WCA50 form we will start to prepare on new form. Thank you very much

ESA50 form has been replaced by UC50 form, they are both Work Capability Assessment forms, I am not sure if CB ESA claimants receive ESA50 review form or UC50 review form.
Review forms are usually smaller than the original forms, we advise claimants rather than state no change, if you kept a copy of your original form use that as a template and make any relevant changes, always number your pages; 1 of 9, 2 of 9 ......... 9 of 9 also add your name and NINO to each page.

So we want to double check to draft out a form ready if it relates to us. If you are on ESA Contribution based support group you now fill in this form? Appreciate your time and help just asking impossible to complete when arrives in the time frame required to so prepare ahead of time
