DLA Renewal Form

Hi I have just got my 1st Renewal pack through for DLA and it is a 39 page application! I have read that most people seem to be getting a 4 page one? anyone else had this long application ???

Whether you receive the 'long' DLA form or the 4 page version, can depend on the area of the country in which you live.

The 4 page version is still I believe being piloted in only certain areas, but at least the 39 page form gives a chance for the claimant to give details about their medical condition/disability, in a much more comprehensive manner than the short form permits.

Hi, I received the 4 page form a week ago and just filled in 'no change' instead of the full facts again that this site advises. Should I ask for another form and start again, or just let sleeping dogs lie? How likely is it for my claim to be thrown out, as this site says is possible?
