PIP 'How your Disability Affects you ' form

Hi,
I have been awarded both daily living and mobility care enhanced rate from December 2013 until April 2016.
My award letter states that they will contact me after March 2015 to make sure if I am getting right level of PIP. This letter also states that if my PIP has an end date then (it has) I will receive a letter around 14 weeks before my awards ends .And this new letter will explain how to claim PIP again.
Today I've received a letter and 'How your Disability Affects you" form.
I need to fill and send it back in a short time. I am fully of worries and already feeling veey stressed!
Letter says that after I sent my form back ; they will decide if I need a face to face appointment or if they can make the decision without having one.
What I don't understand is ; they will contact me around January time (14 weeks before my awards ends)
If I get a face to face appointment at this time ; I will have another one 8 months later in January 2016.
I do think it's not fare that I have to fill the forms and possibly have a face to face within 8 months twice.
Is that normal?
My current (regular one that who knows my conditions and health better than any other doctors at my GP is currently away.
The letter I received today says that 'please send any medical reports, letters etc.' I am panicking that if my GP doctor won't be back before I send this form back.
When I received my award letter I thought 'they will send me a simple letter to check my condition etc. I was NOT expecting a new form with so many questions and possible face to face appointment.
What shall I do? Is that all normal? Please I really need your advice.
My condition has gone worse . I am taking lots of medication . Still under regular care of GP and Hospital. I am Many thanks in advance.

A number of other members are also going through this process at the moment, unfortunately none have at this time completed it, so we cannot tell you what will happen in regard to the "14 week" assessment or even how they will deal with the results of this one.

You should treat this is a full re-assessment of your claim so you need to complete the PIP2 form in detail and supply evidence as you can. Please be aware that whilst the Descriptors have not changed, some of the associated guidance has so you should look at the current PIP Claim guides for assistance in completing the form.

Hi Gordon,
Letter that I received today says 'looking at your PIP again'.
When my award letter says 'we will contact you after March 2015 to check if you are getting right level of PIP I did not think that I would have to complete whole 'how disability effects you' form again.
It's like applying for PIP all over again.
As I stated my award has an end date . They will again send me a letter 14 weeks before my award ends . (That will be around January 2016.
I am very confused and stressed that I will have to fill PIP form twice in 8 months time. (Now and before 14 weeks of my award end date).
I am very sorry if I am repeating myself.
Has anyone else received 'looking at your PIP letter with 'How disability effects you form' together about a year before their award ends?

Yes, we are rather alarmed by this but it is too early to know what is going to transpire. As Gordon said, we do have some members who have received the same form 1 year before their award is due to end, but no-one has completed the reassessment process yet.

So to make clear they are giving people full pip assessments one year before award end date under the guise of a pip review?

Reason i ask, a cousin of mine (diagnosed 2 years ago with cancer, approx 4 yrs outcome life expectancy)
Has been moved from DLA to PIP, awarded PIP April 2015 for 2 years, Enhanced Living, Standard Mobility.
DLA Award runs out August 2015.
The PIP Letter says he will be reviewed/looked at, re-assessed in March 2016, thereabouts.

(and yes he has asked for a mandatory reconsideration for the Mobility rate)

Hi again ,
As I previously mentioned that my doctor who knows me and my health well is away and my surgery don't know when she will be back.
I have been going to GP regularly and I am on so many repeat prescriptions. My question is ; is medical report from GP good to send with my PIP form? Will it be good to send on its own or will it be better with personal letter from my surgery . (It will then have to be any other doctor at my surgery because my doctor is away )
Can I send my repeat prescriptions copies too?
