If it has been some time then it is acceptable to be less precise, for example; early 1988 would be OK.
It should not matter if the date if different from your previous DLA claims, just explain that you cannot remember.

I did on my pip application on section 15 requests a home visit due to my limited mobility along side other issues with travelling... Does this mean I have to wait for the details of the appointment to arrive then ring them if it's not a home visit?
Having a medical is my worst nightmare as previously I had one and was scored 0 points and had to go to a tribunal which took 6 months to get my DLA back!
I find it maddening because surely all this info is on my records and I went above and beyond including in my application letters from my GP and CONSULTANT, CARERS LETTER, LIST OF ALL MY CONSULTANTS and Health professionals ( 7 in total )
I just can't believe this is still not enough information.!!
I can't find where my local capita centre is ... Is there a way of finding out? Cause there's no way I can travel 90 mins I can't even travel 30 mins in a car anymore.

This is likely a standard letter the Capita send out when they receive a claimants file from the DWP, Capita do the majority of their assessments in the claimants home.
Do any of your letters specifically state that you cannot attend at an assessment centre, if they do not then you should speak to your GP and ask whether they will provide you one.
I'm afraid that Capita do not appear to publish the addresses of the assessment centres
