Make it a process.
1. What am I disagreeing with
2. What does the report say
3. Why do I think it is wrong
4. why do I think this
5. Where is the information to support what I am saying
So
1. I disagree with the decision not to award points for Preparing Food
2. The assessor has referenced my ability to climb stairs as a reason not to award any
3. No account of my dexterity problems have been mentioned
4. Problems with my hands mean I can't cut up food
5. See my answer on the PIP2 for further details and the evidence on page X of the information attached to it.
Obviously you should try and expand on this more than I have. It's also an approach to dealing with the MR in general, create your bullet points and then go back over them to expand them in to sentences and then paragraphs if you need to.
Try and break the PIP activities down into tasks, so preparing vegetables might be; washing them, peeling them, cutting them up, putting them in a suitable pan to cook, cooking them, removing them from the pan when cooked. Then you can explain the difficulties you have with each task.

Thanks for your reply. Your examples certainly make sense. I only wished I could express my words and write them down in such a professional manner the way you do.
I will endeavour to the best of my ability, and try to make some headway / progress with the report.
