At time of assessment I had not prepared any proper meals for about three weeks. I am more likely to eat if someone makes a meal for me but even then I don't always finish it.
Based on what you have posted you are saying that you need prompting.
Supervision means that you need someone with you as there may be a danger to you or others, for example you start cook food and then forget that it is on.
Assistance means that you need someone to help you with part of the activity, for example you can prepare and cook the meal but cannot lift the pans to and from the hob (and an aid would not help).

I don't think there is any value in your stating the number of points, the two; the Descriptor and the points are intrinsically linked, if is accepted that you meet the Descriptor then you get the points, the DM has no flexibility in this.
