The PIP2 asks you to detail your medical history, did you not complete this section?

Yes I did Gordon, sorry I had confused my self while trying to break down and understand the decision makers statement. All they had to go on at that point was my say so, seems they accepted my conditions just not how my conditions affect me.
The court has pushed back assigning me a date for Tribunal to Dec 25th
I am struggling to find any new supporting evidence to send with my submission because my conditions are so longstanding in nature, all I have are current specialist reports.
Do you think that will suffice?

Are you sure you mean 25 December?

It's not always a question of providing new evidence, the most important thing is to make sure that you have explained how you meet the criteria for an award but one place you can look is your medical records at your GP Practice, the Consultants, etc. that you have seen will normally write to your GP with the details of any tests and meetings that they have had with you.

Gordon an update by Dec 25th, not actual tribunal date ;)
I did get the last 2 years of gp notes, just a couple of pages of tiny print, nothing of worth in there apart from a load of INR compliance entries and routine unrelated appointments, seems the specialist letters from the joint assessment clinics and reviews I attend will have to do.

Unrelated to PIP tribunal I have also used a right of access request, out of sheer curiosity, to see all the information they hold on me regarding my old DLA claims from 2001-2017.I had never submitted any extra info myself to those, but they must have gathered some that enabled them to award me an indefinite high rate at the time.

Hello, a Submission post yet again!
Planning and following a journey
As someone who can see a row of shops about 200 meters from their house but has not been able to walk to them in numerous years do you think this would fit in any shape into this category?.
Under DLA I had high rate mobility, now I'm wondering if a theoretical score of 10 for the moving around component would be all I could realistically hope for if lucky at tribunal.

Thanks

The Going Out activity looks at three things, it can't consider any physical problems unless you can show they result in the following.

Planning a route - this is primarily a cognitive or sensory (e.g. blindness) activity. You are being asked about the problems you would have with working out how to get from one place to another, you do not need to be able to follow the route that you are planning.

Undertaking a Journey - this is to do with mental health issues such as agoraphobia and social anxiety and is concerned with you leaving the house to go somewhere, they will be interested in the things that stop you doing this. You need to show that you would suffer "overwhelming psychological distress" to meet the criteria.

Following a route - This activity about the problems you would have navigating a route. So are there problems; cognitive, sensory or mental health issues that would prevent you from doing this? This is different from undertaking a journey, in fact, if you cannot undertake a journey then you will not score points for following one and vice versa.
