1. I'm really sorry but I don't know what you mean by "includes colour" could you explain further.
2. Yes, you can provide your answers separately from the form, just make sure that each page has your full name and NIN at the top and that they are numbered, we recommend "page X of Y" so that it is easy to see if pages are missing. Make sure you clearly mark on the form where your answers are and on the relevant page that you are answering question X.
Attach the pages to the back of the form using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.

WRT to includes colour - it's literally a diagram that is colour coded which helps better explain my symptoms.. I know this sounds unusual but it was the best way to communicate some issues. I guess I'm wondering if the colour would get lost before the decision maker gets to see it as they scan stuff in??
Bulldog clip would be OK presumably if well packed for post? I don't have those other things...

Thank you for explaining, all of the stuff I have seen from the DWP suggests that they scan in black and white so some colours may be lost and some may appear the same as others, could you provide an additional key over the top, for example; could you add a number to all sections that are related by the same colour?
A bulldog clip should be fine providing there are no issues getting it in the envelope.

Is it possible to request for a the review to be done on paper? Plead even?
It's looking like I'll be recovering from some unpleasant surgery around the time they get to it.
I doubt I'll be in a good state.

You can certainly ask but the reality is that a Paper Based Review will only be done if you are able to provide sufficient information in and with the form to allow one to be carried out.
They can consider situations where doing a face to face would be harmful to the claimant but this will require medical evidence to support the request and it is unlikely that they will consider anything that you might send now, in advance of the surgery but I would tell them about it on or with the form and provide more information when you can.

Extremely distraught.. did everything I could to give all the possible answers in my review for DWP to make a paper decision, including asking for one, providing corroborating support letters gp, support worker, tribunal decision notice, cert of ssevere mental impairment, consultant letters, diagnosis, ESA85A, all corresponding exaclty with previous PIP2 ...
Called yesterday and after a while was case manager called me, we chatted about 15 minutes he said he had enough information and would sent out a letter 5-7 days.
BUT he phoned later and said he's not medically trained so I now need an assessment? Also when asked said he's a decision maker (In reality I think he's a case manager). I pleaded for him to call my doctor, call my psychologist, ask me anything to get it done... he just wouldn't .. wouldn't say what part of the form he couldn't answer, wouldn't give details on which office he works in..
Went from relief (that it was a paper assessment) to lost in depression. My Tribunal was only last year (a judge and two doctors deciding based on the weight of evidence) and my main issues won't have changed autism is a permanent lifelong disability
I've asked for a callback from a decision maker, is there anything I can do? Can I request another decision maker to look at my case? Put in a complaint? I can't contact my MP parliament is dissolved..
He did say he could ask the provider (ATOS or whoever it is now) to do a paper assessment but as I understand it paper assessment with those agencies is likely for them to do pretty much anything regardless of evidence.. the success results are much lower right?

I am already extremely anxious about terrifying surgery which will probably happen around the same time (mentioned to DWP).. suicidal ideation already..
