Hello Gordon, Should i list the side effects at Q13 anything else you think we should know about your calim or add them to the first extra sheet after listing all of the mediations?
I had to atsrt an extra sheet as couldn't even fit all of illesses on from let alone all of meds.
2, Will the decision maker have a copy of the original PIP calim from from 2017 to refer to or just the aeessors report and the previous award decision letter?
3, Is it correct that the review form goes to the decision maker first to see if they can renew award without sending it to Capita?

1. If the medication has a general effect then list it under Q13 but if it has a specific one then you should include the information as part of your answer to that activity.
2. Yes, they should.
3. It does but only a handful of claims will be decided by the DM at this stage.

I have read conflicting advice about how to fill in the AR1 form. I have seen to treat it as aPIP claim form and at other places to just write whether you continue to have problems with an activity due to your ongoing illnesses. If they have got gradually worse eg Fibrromyalgia more fatigue and pain poor concentration and focus so cannot cook and also the safety aspect of self harm with sharp implements in the kitchen and issues eith side effects of strong pain killers and psychiatric meds.
I don't know how much information to give on the forms, basically B/F can't prepare and cook meals due to severe fatigue & pain and the inability to concentrate to follow recipes and time stuff, not burn pans and forget what hes's doing, urges to self harm with knives, sissors etc.
aI seem to be writing an essay it's to long. Will they already have access to all of the info on the PIP claim from from 2017? Do I need to writ reams of stuff? I'm running out of time and only on question 1 Please help.

When filling in the AR1 form, what you don't want to do is just write "the same" in each box Even if it is the case that there has been no change since the previous PIP award you want to say so in a couple of sentences (highlighting the key issues) as a reminder to them. If something has got worse - say so. This is the advice we give in the guide. You don't have to write reams and reams of stuff - just paint a clear picture of where the claimant is at the current time.

Hello, Just got a question re review form...B/F Fibromyalgia and severe fatigue has got gradually worse as has hie mental health with a recent new diagnosis on top M/H wise. My question is I have answered Question 1 Preparing and cooking food in great detail (maybe to much) as it has had a fairly big impact on his ability with that activity as I have to do it all for him. BUT for the activity eating and drinking the worsened condition has not had any change to his ability to eat and drink....So for activities were the worsened illnesses have not really made a difference to his ability do I have to explain the Changes on the form in the Tell us if something has changed box? Or just say I continue to suffer etc and don't go into details of the change in his condition on those questions as it has not had any change of impact since the original PIP claim form 2 years ago. Hopr I've made it clear bit convoluted I know but am running short on time to send from back for various reasons. Thanks.

On the eating and drinking - say the ability is unchanged, but do reiterate even if only in a couple of sentences - 'what hasn't changed.'
