Hi D65,

If a face to face took place, it's an ESA85 Medical Report, if no face to face, it's an ESA85A.

There will also be an LT54 Form filled in by the DM.

To be sure, you could also pose the particular questions, i.e. how long is the prognosis of the SG award in question, (what is the recommended date for your next reassessment) and which SG Descriptors where applied in placing you into The SG.

Address the SAR : FAO The Data Controller, at the Benefits Office dealing with your ESA.

It may help if you write SAR DPA 1998 in the front, top, left corner of the envelope.

Sending Documents to the DWP or ATOS

Once received, they have 40 calendar days to comply.
