The decision to do a paper review will be dependant on your providing enough detail in and with your ESA50, they can only do one if they believe they have sufficient information.

It doesn't matter if they are dated in advance of the claim.
They don't need to mention the DWP, assessment decision are made by Maximus.
We haven't seen any telephone assessment for several years, it's not a route you want to go down, unless they have changed significantly.

A letter not to have to attend at the Job Centre is not going to get you into the Support Group, the requirement is far more detailed than this, so you should check this in the guides before you approach your GP, however, I do think it is a good idea mentioning both requirements when you see them.
