I forgot to ask about the face 2 face does it antagonize the assessor if you ask their credentials and if your Carer takes notes? I have 2 dictaphones the same but I believe they only allow the ones the police use which are the size of a video recorder!

If you mean their experience, then you can ask but expect them to say Doctor, Nurse, Physio, etc.
Your companion can take notes.
You can record the assessment providing you have written permission in advance, the Dictaphones must use tape, e.g. not digital but otherwise would be acceptable.

If you send a prescription print off with your form do you still list the medications on the form?
If no, where would you write the side effects of the medication?

So put the side effects on the pip form but don't list items again if you are sending a prescription list?
