Health Assessment Advisory Service was created by vmeo
Hi, can anyone give any information about the Nottingham Centre for Health and Disability Assessments, we have just received from them form ESA50 04/15, we have never heard of them, our last form was dealt with by the DWP, we have to complete the form and return it to them by the 9th December. Thank you.

The Centre for Health and Disability Assessments also knows as Maximus is the Assessment Provider for all ESA assessments on the UK mainland. they replaced the former AP, ATOS last year.

Thanks for the speedy reply, my husband was placed in the support group in 2013 having transferred from Incapacity benefit which he had been in receipt of since 1992, the form came as a surprise because there was no end date on the award. We'll just have to fill it in and wait then.

Whilst ESA is an indefinite award the normal maximum period between assessments is three years.
Please make use of the current ESA Claim guides there may have been changes to the ESA Descriptors and Regulations since you were last assessed.
