Subject Access confusion

Phoned the DWP. Given an address in Telford for a Subject Access request, for all info held. Is this correct? Could not find on Gov.UK. Is there a template?

Should I not also write to PIP decision in Wolverhampton, to request all info that was used to make the decision? Is there a template?

Couldn't find the above on the Gov.UK site...

Please do not post duplicate messages, there will always be a delay before they are published, even when mods are officially 'on duty'. How long this takes depends entirely on how many (voluntary) mods are on-line and how busy we are.
