My address is the same (I was living away for University when I was last assessed for ESA but I had to withdraw because of my condition so my permanent address/home has always been my parent's home) so that's ok.
Re. the GP note- I actually just meant a letter of support from my GP, not a sick note. My Dad phoned the GP practice the day before I submitted my form and they said that they do not provide GP letters for benefits claims anyway (not sure why they said something different last week!). I'm now just really hoping that the letter from my Social Worker is enough.
My Social worker also advised that she felt that having to attend an assessment would be likely to worsen my condition- do you think that the assessor will take this into consideration when deciding whether or not I should attend an assessment?

They may do, but from what you have posted, the implication is that there is no actual restraint on you attending and the possibility of you being affected by doing so is not certain, so I think it likely that it will be insufficient.
